HOW TO REPORT A DMCA NOTIFICATION

IF YOU ARE A CONTENT OWNER FROM THE UNITED STATES OF AMERICA:

The following is the process set out under the United States Digital Millennium Copyright Act:

Content owners or an agent wishing to submit a SIGNED notice to us of possible infringement of owned content must either:

i)              scan and submit it to us at DMCAAgent@Utherverse.com;

ii)             or you can fax it to: (1) 604 357-5214

iii)            or mail it to: 659 Columbia Street, New Westminster, BC V3M 1A8

YOUR SIGNED COMPLAINT must contain the following information:

(i) A physical or electronic signature of a person authorized to act on behalf of the owner of an exclusive right that is allegedly infringed;

(ii) A clear description identifying the location where the work or works claimed to be infringing is situated (which world, which region, which room etc.);

(iii) Identification of the material that is claimed to be infringing;

(iv) Information reasonably sufficient to permit us to contact you, such as an address, telephone number, and an email address;

(v) A statement that you have a good faith belief that use of the material in the manner complained of is not authorized by YOU or if you are the authorized agent, then by the content owner, its agent, or the law; and

(vi) A statement that the information in the Notification to us is accurate, and under penalty of perjury, that you are the owner or authorized to act on behalf of the owner of an exclusive copyright that is allegedly being infringed.
